Collect documents from participant *Appendix A
Key in applications to MM2H portal https://www.mm2h.gov.my/login
Original documents to be submitted to MOTAC within 3 days of online application before 10am (Payment – 20% of agency fee from applicant)
Upon approval, submit documents (*Appendix B) to the MM2H portal Pay Government participation fee RM200,000 (payment slip to attach)
Original documents to be submitted to MOTAC within 3 days of online application before 10am (Payment – 80% balance agency fee from applicant)
Once approved (3–6 months), pay the remaining 80% within 14 days.

APPENDIX B
After Approval
- Copy of Conditional Approval Letter
- Security Bond (Stamped by LHDN -RM10.00) (Including)
- Health Insurance Policy – Applicable for age 60 years and below (including)
- Medical Report for Principal and dependants must undergo Medical Examination at Panel Clinic / Hospital
in Malaysia (Including) - Fixed Deposit (FD) Certificate & Bank Confirmation Letter
- Copy of Payment Receipts / Statement for
i. Security Bond ii. Process Fees iii. Fixed Deposit - Statutory/ Oath Declaration of Single Status and Unemployment in Malaysia (For dependant age 21 – 34 years old)
- Original Passport of Principal and all dependants
APPENDIX A
- The Principal to provide a CV with financial & employment details
- Latest Copy of Principal Applicant’s Passport (Biodata & Signature Page). Passport must be valid for minimum 2 years
- Latest Copy of Principal Dependant’s (Passport Biodata & Signature Page). Passport must be valid for a minimum 2 years
- Passport Size Photo 35x50mm with blue background for Principal & Dependant, 4 copies for each pers
- IMM.12 Form (Two Copies) Immigration Pass Application Form Malaysia My Second Home
- Letter or Certificate of Good Conduct (CGC) / Police Certificate / Certificate of Clearance (Applicable fo and Dependant Age 18 Years and above)
- Proof of Family Relationship / Marriage Certification / Birth Certification (Translated to English and Cert Malaysian Embassy / High Commissioner / Consulate or at The Malaysian Ministry of Foreign Affairs)
- Other necessary documents, if needed
- Translation & certification by Embassy (China only)
Other English document, only need to be certified by Embassy
